Report a Hazard

See Something Say Something!
According to OSHA, one of the "root causes" of workplace injuries, illnesses, and incidents is the failure to identify or recognize hazards that are present, or that could have been anticipated. So, what should you do if you notice a hazard?
As an employee, all health and safety hazards should be immediately reported to your Supervisor, Safety Committee or Human Resource Office. In fact, OSHA requires employees to report workplace hazards to their supervisor and promotes open communication about workplace hazards. Coos County believes that safety is everyone’s responsibility and encourages anyone to report all potential safety or health hazards.
All hazards reported using this webform are entered into a secure database. This procedure allows for prompt reporting and proactively assists Coos County Safety & Risk Management Program to control hazards and implement preventative measures to protect our employees.
Please note: reports entered via this webform DOES NOT count as part of emergency communications to triage an incident while it is happening. This online hazard reporting process is SOLEY intended to provide a platform for Coos County Employees and Residents to report hazardous conditions or practices when they are noticed.
